The following table provides a general description of each of the services that
are contracted by the Franklin County Office on Aging Senior Options program. For
more specific information, see Conditions of Participation
and Service Specifications.
Adult Day Services
|
Adult Day Service is a community-based program designed to meet the needs of functionally
or cognitively impaired adults. Adult Day Service consists of structured, comprehensive,
continually supervised components provided in a protective setting that are delivered
based on individualized care plans. Required components include health services,
client activities, personal care services, meal services and transportation. Optional
components include social work services, rehabilitative services, physical therapy,
speech therapy, occupational therapy and bathing. A unit of service is one day of
Adult Day Service.

Biohazard Cleaning Services
|
Biohazard Cleaning Services are designed to improve, restore, or maintain a clean,
sanitary, and safe living environment through the removal of hazards posing a threat to
the client’s health and welfare. Biohazard cleaning services are provided only in cases
where neither the client, nor anyone else in the household, is capable of performing or
financially providing for the services, and where the FCOA case manager has determined that
no relative, caregiver, landlord, community/volunteer agency, or third party is capable of or
responsible for the provision.

Durable Medical Equipment (DME)
|
Durable medical equipment includes equipment designed to increase client safety
and independence. A unit of service is one DME item delivered and/or installed to
the client’s home. This shall include instruction and set-up.

Emergency Response Systems
|
The Emergency Response Systems (ERS) service is designed to monitor client safety
and provide client access to emergency services through the provision of an electronic
communication system. A unit of service is one month of rental for an Emergency
Response System.
|
Extermination/Pest Control
|
Extermination/Pest Control means a service that improves, restores, or maintains
a clean, sanitary and safe living environment through the performance of tasks in
the home that are beyond the client’s capability and the removal of pests posing a
threat to the client’s health and welfare.

General Cleaning Services
|
General Cleaning Services are designed to improve, restore, or maintain a clean, sanitary, and safe
living environment through the performance of tasks in the home that are beyond
the client’s capability. General cleaning services are provided only in cases
where neither the client, nor anyone else in the household, is capable of performing
or financially providing for the services, and where the FCOA case manager has
determined that no relative, caregiver, landlord, community/volunteer agency,
or third party is capable of or responsible for the provision.

Homemaker
|
Homemaker services enable a client to achieve and maintain a clean, safe, healthy
environment; assist the client to manage personal appointments and day-to-day household
activities as authorized by the case manager. A unit of service is one hour of direct
client service.
|
Incontinence Supplies
|
The Home Delivered Incontinence Supply Service is designed to promote the cleanliness,
good health and safety of clients experiencing incontinence. This is to be accomplished
by the provision and home delivery of incontinence supplies. A unit of service is
typically one case or package of any given incontinence supply delivered to a client’s
home.

Home-Delivered Meals
|
The Home Delivered Meal service is designed to promote adequate client nutrition
through the provision and home delivery of one or two safe and nutritious meals
per day. A unit of service is one meal prepared and delivered to the client’s residence

Minor Home Repair, Modifications, and Maintenance Services
|
Minor Home Repair, Modifications, and Maintenance Services provide environmental
accessibility adaptations to the structural elements of the interior or
exterior of a client’s place of residence that enable the client to
function with greater independence in the home and remain in the community.
Modifications, maintenance and repairs that are excluded from this service
are those adaptations or improvements to the home that are of a general utility
and not of direct medical or remedial benefit to the client, such as
carpeting, roof replacement, and adaptations which add to the total square
footage of the home, etc.

Moving Services
|
Moving Services is the moving of a client’s furniture and belongings to another location within
Franklin County. This service also includes the pickup and delivery of furniture from the
Furniture Bank of Central Ohio.

Nutritional Supplements
|
The Home Delivered Nutritional Supplement service is designed to promote adequate
client nutrition through the provision and home delivery of nutritional supplements.
A unit of service is one case of nutritional supplement delivered to a client’s
home.
|
Personal Care/Respite
|
Personal Care/Respite (PC/R) services enable a client to achieve optimal function
with Activities of Daily Living (ADL) and Instrumental Activities of Daily Living
(IADL) and/or provide the client's informal caregiver a respite from care-giving
duties. Personal care assistance may include help with personal hygiene, grooming,
foot care and moving about the home. All services will be provided in the client’s
home of residence. A unit of service is one hour of direct in-home service to the
client.

Transportation
|
Medical Transportation (lift and non-lift) includes transportation for medical appointments and transportation to medical facilities and pharmacies.
Expanded transportation (lift and non-lift) enables clients to travel to various locations within Franklin County to assist in meeting their social and personal needs.
Escort transportation (lift and non-lift) is for Clients who have no available companion to assist them during a trip.
One escort (for purposes of assisting the client) may ride with the client without charge to Franklin County Senior Options (FCSO).
Providers are not required to transport more than one escort per client.
A unit of service is one mile for medical and expanded transportation and one hour for escort transportation.
